What is Care Coordination?

Care coordination refers to the process of organizing and managing healthcare services for a patient in a way that is efficient, effective, and patient-centered. For example, at The Lanby, the Concierge Manager is the  care coordinator who works with a patient's physician, wellness advisor, and specialist doctors, to ensure that the patient receives comprehensive and coordinated care.
