Dr. Indira Gjoni

Dr. Indira Gjoni is a board-certified family medicine physician with a BA from Johns Hopkins University, an MPH in International Health from Columbia University, and an MD from St. George’s University. She completed her residency in Family Medicine at HackensackUMC Mountainside and has received additional training in Functional Medicine through the Institute for Functional Medicine.

Sylvia Chudy, MD

Dr. Sylvia Chudy is a Primary Care Physician double board-certified in Family and Integrative Medicine. She graduated from Poznan University of Medical Sciences and completed her Family Medicine residency at Northwell Health. Before joining The Lanby, Dr. Chudy served as the Lead Physician at Crossover Health.

Brandon Caraballosa, RD

Brandon is a Registered Dietitian and Personal Trainer. He received his Bachelor of Science in Nutrition from Long Island University and Master of Science in Clinical Nutrition from New York University. His blended background in nutrition and fitness contributes to his whole health approach.

Taylor Fazio, RD

Taylor received both her Bachelor of Science in Nutrition and Dietetics and Master of Science degree in Clinical Nutrition from New York University. As an all-around-wellness expert, Taylor takes an individualized approach to each member's guidance on nutrition, movement, sleep, and stress-management.

Rachel Brief, RD

Rachel is a Registered Dietitian Nutritionist with a passion for holistic care. She earned her master’s in Clinical Nutrition and Dietetics from NYU and a Bachelor of Science from the University of Maryland College Park’s School of Public Health. Rachel brings an empathetic and tailored approach to counseling.

Alexandra Rooker, NP

Alexandra is a board-certified Family Nurse Practitioner with a passion for skin health, longevity, and personalized care. She earned her Master of Science in Nursing from Georgetown University and has gained experience across diverse clinical settings, including several years in Hawaii.

How Membership Works

Onboarding Call

  • A 20-min call with your child’s Member Advocate. 
  • Learn what’s to come and how to maximize their membership.
  • Share your preferences and goals to ensure we create the best possible experience for your child and family.

Comprehensive Annual Baseline Visit

  • 80-minute in-person session with your child’s Physician and Registered Dietitian.
  • Review of your child’s medical history, diet, behavior, digestion, sleep, and level of activity.
  • Discussion around parents’ support network, current needs and long-term goals.
  • Comprehensive physical exam and labs based on your child’s age, health history, lifestyle and symptoms.

Personalized Care Plan

  • A 30-minute follow-up visit to review the personalized Care Plan developed for your child.
  • Clear next steps across nutrition, routines, supplements, and advanced diagnostics.
  • Guidance and coordination of medications and specialist referrals.

Ongoing care

  • Quarterly check-ins to address new concerns proactively and ensure progress.
  • In-app chat or text for support between visits.
  • Same-day visits for acute issues or deeper guidance.
